Unless the National Historic Preservation Act (NHPA) Section 106 process has already been initiated with Tribes, the State Historic Preservation Office, or other interested parties, do not initiate the NHPA Section 106 process without first consulting with the Rural Utilities Service (RUS). The Site and Route Environmental Questionnaire (EQ) is intended to capture the appropriate environmental information for each proposed site and route included in the application. This information will be used in conjunction with the Project EQ to determine the overall environmental compliance of the proposed project. RUS will use this information and either complete the environmental review process or determine the appropriate level of additional impact analyses needed, in accordance with 7 CFR 1970: Environmental Policies and Procedures. RUS will notify applicants if additional information is necessary beyond what was submitted in this document.

Site and Route Environmental Questionnaire

The NHPA Section 106 EQ is intended to capture the appropriate information for each proposed site and route included in the application, as well as the overall proposed project. This information will be used in conjunction with each Site and Route EQ and Project EQ to determine the overall compliance of the proposed project with NHPA Section 106 requirements. RUS will use this information and either complete the NHPA Section 106 portion of the environmental review process, or determine the appropriate level of additional impact analysis needed in accordance with 7 CFR 1970: Environmental Policies and Procedures.

National Historic Preservation Act Section 106 Environmental Questionnaire

The ReConnect Program Network Design Certification certifies the proposed network will be designed and constructed to deliver broadband services to all the premises in the proposed funded service area (PFSA, as well as that, as designed, the network can meet the proposed build-out timeline, milestones and construction schedule within the costs specified in the application.

This document certifies your proposed network as one that has been designed and constructed to deliver broadband services.

ReConnect Program Network Design Certification

All applicants applying under the ReConnect Program or Community Connect Grant Program are required to submit an Authorized Representative Request (ARR) and a resolution in order to access the online application and online financial reporting and compliance (R&amp;C) system. The resolution – or its equivalent – is dependent on the legal structure of the applicant. Sample resolutions for corporations and LLCs are included on pages 2 and 4 of this document. If the applicant is an entity other than a corporation or LLC, the content in the sample resolutions can be modified to reflect the applicant’s legal structure.
